I have done all of this (below) 5 times on a new brain out of the box.

Clear and Re-upload Programs: Delete or clear all stored programs from the V5 Brain, then re-upload your compiled program fresh from VEXcode V5. [1, 2]

Update Firmware (VEXos): Connect the V5 Brain to your computer via USB and open VEXcode V5. Check the brain icon in the top right; if it is orange (out of date), click it to update the VEXos firmware to the latest version. [1]

Check Slot Selection: Make sure you are downloading the program to a valid program slot and selecting that same slot on the brain before running it.

Reset Brain Settings: If the error persists, you can restore default settings by going to Settings > Reset settings on the V5 Brain screen, though this will wipe customized preferences. [1]
